8/14/2023 EnchantaVaughn Approved Behavior: JaCarei greeted QMHP-C as QMHP-C arrived to the home. JaCarei appeared to be in a positive mood while discussing with QMHP-C how his day was going. He verbalized to QMHP-C that his day was going well and that he had a good day. QMHP-C engaged mother in a discussion to determine behaviors since last session. Mother verbalized to QMHP-C on how JaCaeri is struggling with managing his anger which causes him to get in trouble alot at home. Behavior: JaCarei needed redirection from QMHP-C throughout the second portion of the session as he was displaying off task behaviors. Once he was redirected by QMHP-C he was able to regather his behavior and get back into the session. He apologized for the behaviors he was displaying. Intervention: QMHP-C engaged JaCarei in self-controlling behaviors as well as how to control his behaviors. QMHP-C role-played with JaCarei on his peer interactions and triggers that causes him to act out inappropriately in the school setting. Intervention: QMHP-C modeled effective ways to express feelings and thoughts accordingly and engaging in respectful behaviors. QMHP-C engaged JaCarei in controlling his frustrations and anger control. QMHP-C taught JaCarei different techniques to assist him with showing feelings rather than acting out the behaviors. Response: JaCarei appeared to be receptive to QMHP-Csâ interventions and services that were provided to him during the session. JaCarei struggles with self-controlling behaviors as he acts out impulsively without thinking of the consequences for his actions. Response: JaCarei reported that he has been influenced by his peers as he would act out inappropriately with others. Through role-playing JaCarei was taught to identify his triggers and verbalize his thoughts.
